How to Apply
1. Read all documents before filling in any information.
2. Understanding the information in item “1”, now complete the application form.
- References you list on the front page are the same ones to which letters are sent.
- Answer all questions on side 2 to the best of your ability.
- Financial information must be from the most recent statement/report
- Income is divided into “applicant” and “co-applicant” to enter individual incomes.
- Each applicant must sign at the bottom of side two.
3. Sending out Reference letters is the applicants’ responsibility.
- Refer to the front page of your application for the references you named.
- Address sign and fill in your contact information on the front side of each form letter. DO NOT COMPLETE THE BACK SIDE. References WILL mail or fax the letter back to The Pines.
- If you rent, send out the landlord reference.
- If you own your home, use the Property tax form. Do not send your bills in lieu of this document.
- Each name on the application must submit a medical reference via their primary doctor.
4. Additional forms must be signed. Co-applicants sign individual forms.
- The Release for background information must be signed by the same person.on both signature lines.
- The Declaration form must be filled in appropriately. Non Citizens must read the entire document and provide all the necessary documentation. Note that each household member must submit a declaration form.
5. Return application, release and declaration forms to: PO Box 7354, Ocean Park, ME 04063
6. Verification materials
- Gather documents that support the information entered on the application form and outlined on the back of the cover letter. Click to download information.
- If you are under age 62, submit verification of disability form signed by the applicant but filled out and signed by doctor.
- This information must be presented before or at the time of applicant’s interview
